Two-step two-stent technique to manage a large gastrocolonic fistula.

چکیده
Gastrocolonic malignant fistulas can occur, although rarely, complicating gastric or colonic neoplasms. En bloc resection of the fistulous tract and involved segments is the first-line treatment whenever possible, with endoscopic treatment an alternative. Endoscopic closure has been described using covered stents [1, 2], over-the-scope clips [3], and atrial septal defect occlusion devices [4]. However, in the colon, covered stents are associated with a high migration risk [5]. We therefore developed a new technique to allow fistula closure, overcoming the risks of stent migration by initially placing an uncovered stent, which is followed by the deployment of a covered stent inside the first stent a few days later and fixation of the two stents to each other using through-the-scope clips. Our patient, a 58-year-old woman, had a metastatic and locally advanced intestinal-type gastric cancer with colonic invasion. After receiving 12 cycles of palliative chemotherapy, she was admitted with fecaloid emesis and a computed tomography (CT) scan showed a large fistulous tract (approximately 5 cm in length with a large orifice) between the stomach and the transverse colon. Although the patient had a good performance status, surgical resection was not considered feasible and endoscopic palliation was proposed. Esophagogastroduodenoscopy (EGD) showed a large ulcerated neoplasm in the greater curvature of the gastric antrum, with a large orifice communicating with an ulcerated fistulous tract. Colonoscopy revealed a stenosis in the transverse colon and contrast instillation allowed characterization of the stricture and of the fistulous tract. An uncovered metal stent (WallFlex; 22–27mm×90mm; Boston Scientific, Marlborough, Massachusetts, USA) was then placed in the strictured transverse colon where the colonic fistulous orifice was located. After 3 days (to allow time for embedment of the uncovered stent in the colonic wall), a similar E-Videos

Subclavian artery-esophageal fistula after placement of a self-expanding metal stent in a patient with esophagogastric anastomosis stenosis
Background: There have been reports on stent-related vascular erosions about patients with benign or malignant stenosis of the esophagus who received endoscopic stent insertion for palliative intention for oral intake. Case presentation: A 61-year-old woman with esophageal cancer located in the middle part of esophagus was treated with esophagectomy. Two years following the surgery, malignan...
متن کاملTranstracheal Esophageal Stent Removal: A Case-Series
Benign esophagorespiratory fistula is a rare but often lethal affection and difficult to cure. Possible treatments are surgery or esophageal stenting but may fail and cause respiratory failure. Two patients with spontaneous esophagorespiratory fistula after chemoradiotherapy for an esophageal malignancy were both treated by esophageal exclusion but esophageal stent were left in place. The esoph...
متن کاملDiagnosis and Management of Ureterovaginal Fistula in a Resource-Constrained Setting: Experience at a District Hospital in Northern Nigeria
During 2007 we were invited at different times to review and manage four women with ureterovaginal fistula following caesarean section performed in different rural hospitals. We describe our experience of a simple technique of diagnosis and management of these indigent patients in a resource-constrained hospital. The condition was diagnosed by the three-swab test in all four patients, and abdom...
متن کاملUreterocolic fistula secondary to a self–expanding retrievable ureteral stent
Self-expanding stents are relatively new in the field of urology and have primarily been used for permanent remodeling of benign or malignant stricture. We are presenting a rare and interesting case of a ureterocolic fistula that formed secondary to placement of an expandable, retrievable metal stent in the ureter. After multiple retrieval efforts, the self-expanding metal stent was finally ret...
متن کاملSurgery of the aortoduodenal fistula: two cases with survival.
We report on two cases of aortoduodenal fistula. The patients underwent the positioning of an aortic stent 1.9 and 5 yearly before, respectively. They complained of fever and abdominal pain and were admitted to the hospital. A stent infection was suspected and an angio-CT confirmed the suspicion. Each patient was brought immediately to the operating room, where the fistula was individuated and ...
